If you would like to nominate an employee from your district for any of the awards that will be given at the annual conference, place a check mark in the box next to the category for which the nomination is being made.  You may nominate employees in more than one category by using a separate form for each nominee. 

This year in addition to Supervisor, Shop Supervisor, Shop Technician, Regular Driver Special Needs Driver and Aide.  We are adding Operations Specialist of the Year.  This category would include personnel with the following type of job titles: router, dispatcher, lead driver, trainer etc.
